Care Navigation includes personalized support for specific health concerns. Whitman-Walker provides counseling around medical care plans and treatment goals to eligible patients. Navigation also includes guidance through services within the health center and outside resources to help patients establish healthy support systems. Care Navigation is available for eligible, adult patients engaged in medical care through Whitman-Walker at 1525 or Max Robinson Center.
