Patient-Centered Medical Home (PCMH)

What is a Patient-Centered Medical Home (PCMH)? PCMH works to improve patient health care by creating partnerships between providers and patients and, when appropriate, a patient’s family. Through this model, patients see improved care by working with a team that helps access quality health care throughout the entire health care system. Patients also become an active part of this process as they learn to manage and organize their own care at the level they choose. Recognizing that patients and families are core members of the care team, PCMH ensures that they are fully informed partners in establishing care plans.

What changes and additional benefits will occur? WWH’s model of care already follows the PCMH model – a team of providers, including your primary care provider, work with you to meet all of your health care needs. With new technology and new communications channels, we are working to further engage our patients in their own care and enhance their care experience.
