The Medical Adherence Unit is here to help identify any barriers you may have to accessing health care and staying on your recommended treatment. We want to empower you to take the best possible care of yourself. We can provide you with the information, education and motivation to make healthy choices so that you can be as independent as possible, even when faced with a serious, chronic illness.
Understanding How We Can Help
We have a team of experts who can help you to stay engaged with your health care and on your treatment, understand your diagnosis and how your treatment works so you can stay as healthy as possible and live your life to its fullest!
Our Nurse Case Managers, Care Coordinators, and Referral Coordinators can help you learn about your diagnosis and treatment. They can also work with you to make sure that you are getting the best results from your care:
- Show you how to take your medications correctly.
- Keep your doctor's appointments.
- Help you get other services you may need.
- Improve your diet for better health.
